Our staff is always ready to help with your scheduling, insurance or anything else you may need during your time with us. We will take extra care to listen to your concerns, answer all your questions and thoroughly explain your condition and treatment options. If at any time you are unable to keep your appointment, please notify us. We will be glad to reschedule your appointment at a more favorable time. We greatly appreciate your time and consideration and look forward to seeing you.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, debit cards, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

We accept most insurance plans. Please call us to verify whether or not we are participating with a particular insurance company. We look forward to hearing from you.
